Privacy Policy
This is Jaclyn Seebach’s (“Seebach”) privacy policy. This document explains Seebach's policies for the collection, use and disclosure of personal information. Seebach abides by applicable privacy legislation. Seebach is a physiotherapist. Seebach operates as an independent contractor to a third party wellness centre. Appointment bookings and meeting space may be provided by the wellness centre. The privacy policies of the wellness centre will also apply to certain aspects of Seebach’s services. Seebach uses a third party telemedicine solution based in the United States to provide remote services, which does not itself collect or retain personal health information.
The Information we collect
Seebach collects information primarily by information actively provided by its patients, or from other parties as authorized by patients.
The types of personal information we collect includes health information relevant to our services, and payment information.
How we use this information
This information is used to provide our services to our patients.
Seebach may use aggregate or anonymous information which will not be linked to identified individuals for various other uses for itself and third parties.
Who we share this information with
Seebach does not share personal information with any third parties except as disclosed in this policy. Seebach may provide personal information to its consultants, subcontractors and professional advisers, and the wellness centre (all which shall be bound by privacy obligations) to the extent necessary to assist Seebach’s patient services.
Security
Personal information is stored in a combination of paper and electronic files. They are protected by security measures appropriate to the nature of the information.
Accessing information
Individuals may review their personal information contained in Seebach files by contacting the Seebach privacy officer. If an individual believes that any of their personal information is inaccurate, we will make appropriate corrections.
General
Seebach may amend this policy from time to time by posting an updated version on our website. If such amendments affect how Seebach uses or discloses personal information already held by Seebach in a material way, Seebach will obtain consent.

Notwithstanding the general terms of this policy, the collection, use, and disclosure of personal information may be made outside of the terms herein to the extent provided for in any applicable privacy or other legislation in effect from time to time, or pursuant to court orders.
